Almost crashed a plane on maiden. Throttle was not responding

I have a Cube Orange w/ADSB in a Turbo Bushmaster 120" plane. Radio control is a crossfire nano rx. It is connected to telemetry port 1. Output channels are crsf tx and rx. We did a range check and everything seemed fine. Using a Mauch X8 Battery Kit – with 2 6s batteries in series.
First flight went ok, the plane was very twitchy, but we had not done any tuning. On the second flight it was better after we reduced the end points of the ailerons and elevator. When coming in for landing the pilot stated that he was giving throttle but the plane was not responding. When it did it was like 100%. We were able to get the plane down with out much damage.
Any help to understand what and why is greatly appreciated.

Any RC fail safe warning showed up ?
What was the flight mode during landing ?
Can you share a log if you do not mind ?

No rc failsafe message on the transmitter that I remember.
I thing the pilot switched to manual to get the plane on he ground.

The throttle failsafe was activated, causing the flight mode chage to CIRCLE.
RC signal likely lost during this period.
